Size form for stitched dress

Please use this form to submit the dress size after you have placed your order/pre-order and you received a confirmation email with an order number which you need to enter on this form along with the size.

MEASUREMENTS: Made-to-Measure. Please provide all measurements in inches. The dress will be made according to your size so please be as accurate as possible when giving sizes.
